The Team
The NWRH Social Workers team practices collaboratively with the wider multidisciplinary team to support:
Vulnerable people experiencing an acute medical event and likely to require lifestyle adjustment.
Inpatient discharge planning.
Identified social care needs during and post their hospitalisation.
Reporting to the manager of Social Work services, our team comprises hospital-based Social Workers providing psychosocial support to inpatients and outpatients at both hospital sites, as well as community-based Social Workers supporting hospital avoidance, wellness, reablement, and groupwork support through Primary Health funded services. We engage proactively with the multidisciplinary team and external agencies to ensure optimal outcomes for patients, practicing collaboratively within the Allied Health team across acute and community services, fostering a culture of shared learning and respect for diverse perspectives.
The Role
The Department of Health is seeking motivated applicants for a Social Worker vacancy on Tasmania's North West Coast. As part of the Hospitals & Primary Care - Hospitals NW Social Work team, you will contribute to developing, providing, and evaluating social work services for hospital inpatients and outpatients and their families, in accordance with the Australian Association of Social Workers' Code of Ethics.
The successful candidate will:
Practice across all clinical caseloads including Medicine, Surgery, and Emergency.
Receive ongoing support and supervision from a Senior Social Worker.
Opportunities for rotation across other acute clinical caseloads may be available when workload demands.
Please note: The successful applicant will be required to work on site, attend agency visits, and participate in a weekend on-call Social Work Service.

Details of appointment
This is a permanent, part-time, day worker position (with on-call), working 41.8 hours per fortnight from 26 May 2025. Hours are negotiable.
Applicants should note that the selection process may be used for subsequent vacancies over the next twelve months.
Salary: $73,834 to $111,364 pro-rata per annum, plus 11.5% superannuation. Salaries beyond $119,290 are subject to qualifications or application through the personal upgrade scheme.

Eligibility
Australian Association of Social Workers (AASW) approved qualification.
Membership or eligibility for membership with AASW.
For overseas candidates, a certification of eligibility from AASW.
