The role involves working as part of a collaborative clinical team, supporting GPs and patients to identify and manage chronic conditions. The ideal candidate will be an AHPRA registered Enrolled or Registered Nurse with experience in Chronic Disease Management and health assessments.
Key Responsibilities
* Prepare care plans and team care arrangements to help patients manage their disease/s
* Empower patients to increase ownership over their health
* Coach patients on embedding health behaviour change principles
* Enable patients to access ongoing care from a multi-disciplinary team
* Assist in early identification of chronic disease by conducting health assessments
* Work with GPs to promote preventative healthcare throughout the practice
Other duties include reviewing patient care plans regularly, managing administrative tasks relating to CDM appointments, and contributing to a supportive working environment.
