About the Role
Enrolled Nurse – Transition to Residential Aged Care (TRAC Unit) at Belmont Hospital. Our 20‐bed TRAC unit lives where acute nursing meets residential aged care. You will support patients who have recovered from an acute episode but can no longer safely return home, working closely with families and carers to facilitate a smooth, dignified transition into long‐term aged care.
Responsibilities
* Provide geriatric clinical nursing care
* Deliver palliative and end‐of‐life care
* Offer emotional and social support to patients and families
* Assist with care planning and transition processes
Qualifications
* Enrolled Nurse with current AHPRA registration
* Strong interest or experience in aged care, gerontology, or palliative care
* Excellent communication and interpersonal skills
* Compassionate, patient‐focused approach
* Ability to work collaboratively within a multidisciplinary team
