* We are looking for individuals with knowledge and experience in mental health assessment, care planning, and contemporary practice.
* You must be eligible for membership or registration with the relevant professional body/AHPRA.
* Prior experience working in a consumer-focused way is highly valued.
About the Role
This position requires a strong commitment to delivering recovery-focused care and working collaboratively with consumers, their families/carers, GPs, Community Managed Organisations (CMOs) and other service providers.
Key Responsibilities
1. To provide assessment, care coordination, intervention, and follow-up services to adults experiencing mental health concerns.
2. To work collaboratively with consumers, their families/carers, GPs, CMOs, and other service providers to ensure safe and effective care.
3. To participate in ongoing learning and service improvement initiatives within the team.
Requirements
* A recognised and accredited tertiary qualification and registration (or eligibility for) with the relevant board via AHPRA, or eligibility for membership of the relevant association, in one of the following disciplines: social work, psychology, occupational therapy, dietitian, or nursing is required.
