On behalf of our client which is a well established Health Service in Darling Downs of QLD (just two hours from Brisbane), we are currently seeking an experienced Social Worker to join a supportive and well established team for on a Fulltime Locum basis for 12 weeks. This role is working Monday to Friday within the Allied Health Team. In this role you will be responsible for providing a high quality, direct social work service to all inpatients and outpatients of the Hospital as required. This includes counselling, access to support services, discharge planning and community follow-up

**Key Responsibilities**
- Deliver personal counselling - helping the individual and carers adjust to and adapt to his/her current medico-psycho-social situation
- Assist clients to determine own necessary future plans and options for future treatment
- Work collaboratively in a multidisciplinary team to deliver quality patient outcomes
- Implement and coordinate a plan for discharge and referral of patients, organising post-discharge home support where relevant and required
- Liaise with other members of the multi-disciplinary care team to assist with patient and family adjustment to their illness and other associated concerns and stress factors with the process of rehabilitation
- Show good professional reasoning and sound judgement in decision making, considering broader implications of decisions, including longer term impacts on patients
- Consistently deliver quality care & service which meets or exceeds customer expectations
- Coordinate and implement family conferences as approved by the patient, to discuss future care options, or needs for change of accommodation, or an increase in community support needs
- Act as a resource for staff obtaining information on community support services, counselling needs etc.
- Demonstrate a commitment to quality and actively participate in the department's quality activities and maintain accurate and up to date records
**About You**
- Completed Bachelor or Masters in Social work
- Current membership or eligibility for AASW
- Effective communication and interpersonal skills
- Able to work collaboratively and effectively in a multidisciplinary team
- Be willing to contribute to the development & promotion of a cohesive & interactive team
- Act as a professional role model when dealing with internal and external customers
- Willing to participate in a continuous quality improvement environment
**Remuneration Package**
$63 p/h + Super + + Staff Accommodation based on availability.
