About the Opportunity
As the Home Visiting Liaison Midwife, you will be the bridge between maternity hospitals and Cardinia Shire's dynamic Maternal and Child Health (MCH) Service, ensuring every family receives coordinated, compassionate, and timely support. This role supports families as they transition from hospital to home. You will coordinate seamless handovers and referrals, build collaborative networks with hospitals, conduct initial phone consultations, provide follow‐up support, and maintain accurate data entry while upholding client confidentiality.
Responsibilities
* Coordinate seamless handovers and referrals from hospital services to the MCH Home Visiting Team.
* Build strong, collaborative networks with public and private maternity hospitals.
* Conduct initial phone consultations with parents/carers to introduce the service and schedule newborn home visits.
* Provide follow‐up support such as weight checks and early assessment post‐discharge.
* Ensure accurate and timely data entry while upholding the highest standards of client confidentiality.
What You Will Bring
* Tertiary qualification in midwifery and current registration as a Registered Midwife.
* Demonstrated experience in midwifery and supporting families in early parenting or related settings.
* Strong communication, negotiation, and interpersonal skills with the ability to present information clearly and professionally.
* Proven ability to make sound decisions using good judgement and experience.
* Knowledge of relevant legislation, policies, and frameworks, including the Children, Youth and Families Act 2005, and awareness of family welfare matters.
* Understanding of multicultural considerations and how they influence family experiences and service delivery.
* Strong computer skills and confidence using digital systems to support service coordination and record‐keeping.
* Ability to work effectively within a multidisciplinary team and collaborate with a wide range of service providers.
* Current Working with Children Check, Victorian Driver Licence, and access to your own vehicle.
