You'll work side by side with clients and families to navigate services like allied health, nursing, and home support.Think of yourself as the connector, advocate, and coach — helping clients stay independent while living life on their own terms.What You'll Be DoingCreate and deliver personalised care plansSupport clients in setting meaningful goals and budgetsManage your own caseload with regular reviews and assessmentsBuild solid, trusting relationships with clients and familiesCoordinate services across health, home, and community careAbout YouYou'll shine in this role if you're organised, empathetic, and passionate about improving lives.Must-haves:Experience in aged care case management (HCP/CHSP preferred)Background in community or aged care service coordinationGreat communication and planning skillsAbility to work independently and as part of a teamAble to obtain a Police Check & NDIS Worker ScreeningBonus points if you have:Nursing qualifications (RN or EN)What's In It for You?
