Support at Home Program Manager (Aged Care / Home Care) South Eastern Community Connect (SECC) is looking for a full-time Support at Home Program Manager (currently known as Home Care) to assist our aged clients in the community.
SECC is a not-for-profit organisation serving the LGAs of Bayside, City of Sydney, Randwick, Woollahra and Waverley.
ABOUT THE ROLE The Support at Home Program Manager is responsible for the strategic and operational oversight of the Support at Home Program (SaH) at SECC. The SaH Manager will lead and manage a team of case managers, ensuring adherence to program guidelines and regulatory requirements, effective resource management, and the overall success of the SaH Program at SECC. This role involves ensuring the delivery of high-quality, person-centred Aged Care services to older adults. ABOUT YOU Experience in case management, client assessments, and developing care plans. Sound financial management skills including resource management, budget development and monitoring. Solid understanding of the Australian aged care system, including relevant legislation, funding models, and quality standards. Good customer service skills, with commitment to person-centred care and consumer-directed care principles. AHPRA Registration - Unconditional. Strong organisational, analytical and problem-solving skills. Tertiary qualification (Diploma or above) in management and/or health-related discipline or community services.
