**About the Role**
We are looking for a Community Support Worker to join our Individualised Community Living Strategy (ICLS) program providing outreach support to clients accessing the service **between the Mandurah to Morley areas**. The service is for clients with a mental health diagnosis who are eligible for a support package. The role of the Community Support Worker is to help the client access their local services and maximise their recovery success.
**Key Responsibilities**
- Provide dedicated support to clients with a mental health diagnosis, focusing on recovery and community living goals
- Coach and encourage independence through daily routines and decision-making
- Assist with cooking, cleaning, and home upkeep, empowering the client to take control of their daily living
- Encourage community access and engagement in local activities
- Support capacity building and self-management of lifestyle and wellbeing
- Work collaboratively with clinical and mental health support teams
- Follow and contribute to individualised support plans
- Thursday and Friday daytime availability **REQUIRED**:
- **Flexibility to travel up to 40 minutes** from home, as required. Travel will be within a 40 km radius of the Perth metro area
**Skills & Experience**
- Certificate IV in Community Services or equivalent
- Demonstrated experience working autonomously with people with mental illness or psycho-social disability
- Passionate about client outcomes and mental health advocacy
- High computer literacy and administration skills
- An unrestricted Western Australian driver's licence and access to a reliable vehicle
- A current First Aid Certificate, which includes the course HLTAID003 or HLTAID011
