Out of Hospital Care Case Manager - Wagga Wagga
Community Services #1 is seeking a Out of Hospital Case Manager to support and enrich the lives of people in our community.

About the role:
* Provide comprehensive initial assessments that are goals and needs focused, and sensitive to the cultural, emotional, and social needs of a diverse community.
* Case manage individual packages in accordance with the OHC Program, including maintaining progress notes, monitoring and adjusting services as required, and overseeing budgets.
* Ensure all case management activities comply with relevant legislation, NSW Health policies, funding agreements, contractual obligations, and organisational procedures.
* Assist the Coordinator to deliver on contractual obligations related to monitoring, evaluating, and reporting on OHC program performance.

About you:
* Demonstrated equivalent Certificate IV qualifications in relevant fields such as Community Services or Health Services and a minimum of 2 years' experience in a relevant role.
* Proven experience in case management, including developing, monitoring, and adjusting care and service plans, overseeing package budgets, and managing client exits from programs
* Be available to travel across the Murrumbidgee LHD, Southern New South Wales LHD and interstate (to Canberra), as required.
* Hold a current driver's licence and your own vehicle.
* Hold a Working with Children Check, current First Aid & CPR, unrestricted Australian working rights
