About the Role
We are seeking a compassionate and dedicated Care Professional to join our team. In this role, you will work closely with clients, their carers, and interdisciplinary teams to deliver person-centred care that supports independence, health, and quality of life.
Key Responsibilities
* Complete holistic assessments and develop care plans in line with client goals and clinical recommendations.
* Deliver and monitor services under programs such as CHSP, Support at Home, QCSS, and NDIS.
* Support clients and carers to navigate care systems, provide informed consent, and communicate feedback.
* Collaborate with internal and external clinical staff for complex needs and referrals.
* Maintain accurate records and report outcomes aligned with funding and quality frameworks.
* Build respectful relationships with clients, families, and community stakeholders.
Required Skills and Qualifications:
* Familiarity with Support at Home, Home Care Package, Commonwealth Home Support Programme.
* Confident using mobile technology (smartphones, tablets, laptops).
* Experience in case management or coordination within aged care or disability services.
* Strong understanding of client-centred and community-based care.
* Ability to work independently and as part of a multidisciplinary team.
* Effective communication and interpersonal skills, including written documentation.
* Cultural awareness and sensitivity, particularly when working with Aboriginal and Torres Strait Islander peoples.
