Magistrates Early Referral into Treatment (MERIT) Case Manager
We are seeking a dedicated professional to join our team as a MERIT Case Manager, responsible for providing assessment, intensive case management, support, counselling and treatment to individuals with drug and alcohol issues.
About the Role
This is an exciting opportunity to work in a multidisciplinary team, engaging with people who require assistance with substance use. As a MERIT Case Manager, you will provide holistic treatment and prevention approaches, applying your knowledge and experience to improve the lifestyle and health of individuals with complex needs.
* Key Responsibilities:
* Assess and develop individualized case management plans, ensuring effective support and guidance for clients.
* Provide intensive case management, including screening, assessment, brief intervention, withdrawal management, contributing to treatment and discharge planning, referral, group work and community follow-up.
* Liaise and work in partnership with government and non-government services, and community groups to ensure comprehensive support.
Requirements and Qualifications
To be successful in this role, you will possess:
* Tertiary qualifications and current registration with Australian Health Practitioners Regulation Agency as a Registered Nurse, Occupational Therapist or Psychologist, or Tertiary qualifications for Social Worker, Counsellor or Welfare Officer with eligibility for membership for relevant professional association.
* Demonstrated knowledge and experience of holistic treatment and prevention approaches applicable to people presenting with complex needs.
* Aptitude for case management and treatment options for individuals and their families with problematic or harmful drug use.
* Leadership skills, with ability to work within a multidisciplinary team or autonomously.
* High-level communication skills, advanced written and oral communication, and competent computer skills.
* Evidence of a current unrestricted drivers' license and willingness to drive/travel as required.

What You'll Be Doing
MERIT (Magistrates Early Referral into Treatment) is a court diversion program that provides the opportunity for suitable local court defendants with drug and alcohol problems to address their substance use, associated lifestyle and health issues. Engaging with people who are before the court with a history of or current drug use, providing assessment and case management that will help to improve their lifestyle and health in an effort to reduce re-offending.
* Selecting and developing appropriate treatment strategies tailored to each individual's needs.
* Providing regular progress updates and reviews, working collaboratively with other healthcare professionals.
* Developing strong relationships with key stakeholders, including clients, family members, and community groups.
