Latrobe Regional Health is looking for Nurse Practitioner (Cognitive Dementia & Memory Service) to join our growing team on a part time (0.4EFT) fixed term basis until 30 June 2026.
The Nurse Practitioner provides advanced clinical assessment, diagnosis and management for patients presenting with cognitive impairment and suspected neurocognitive disorders within the Cognitive Dementia and Memory Service. Working as part of a multidisciplinary team, this role delivers expert dementia diagnostic services, including comprehensive cognitive assessment, interpretation of diagnostic investigations and development of person-centred management plans. The position supports timely access to specialist dementia care, promotes coordinated pathways with primary and community services, and contributes to service development, quality improvement and dementia education across Latrobe Regional Health and the broader community.

The role of the Nurse Practitioner (Cognitive Dementia & Memory Service) is to:
* Provide advanced clinical assessment, diagnosis and management of patients with cognitive impairment and suspected neurocognitive disorders with the CDAMS service.
* Conduct comprehensive cognitive assessments, including clinical history, cognitive screening, functional evaluation, and ordering/interpreting diagnostic investigations within scope of practice.
* Develop and implement evidence-based management plans, providing education, guidance and support to patients and families regarding diagnosis, prognosis and available supports.
* Facilitate referrals and coordinate care pathways with general practitioners, specialists, allied health clinicians, aged care services and community providers.
* Provide clinical leadership with CDAMS, contributing to service development, quality improvement initiatives, evaluation activities and education to support best practice dementia care.
* Work collaboratively with geriatricians, nursing staff and multidisciplinary teams to deliver coordinated, person-centred care that promotes dignity, autonomy and safety.

About you Ideal candidates for this role will have:
* Current registration as Nurse Practitioner with AHPRA and completion of a relevant postgraduate qualification in advanced clinical dementia practice.
* Advanced clinical experience in assessing and managing older adults with cognitive impairment, dementia or complex neurocognitive disorders.
* Expertise in conducting and interpreting comprehensive assessments using recognised tools.
* Highly developed interpersonal skills, with the ability to provide clear education, sensitive communication and support to patients, families and carers.
* A commitment to service improvement, clinical education and integrated care across health, aged care and community systems.
