The Aged Care Clinical Assessor is responsible for conducting comprehensive client assessments for older people within the Aged Care Planning Regional area of Victoria (VIC 3 – Loddon Mallee). This role forms part of the My Aged Care Single Assessment Workforce, delivering high-quality, person-centred assessments that support older Australians in accessing appropriate aged care services.
The position involves both face-to-face assessments requiring travel across the region and remote assessments via telehealth or phone. As a clinical member of the assessment team, the role also provides clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors, ensuring assessments are accurate, holistic, and aligned with relevant aged care standards and policies.
We are seeking staff who are primarily interested in working in our Triage area, with the potential to conduct comprehensive assessments via telehealth as needed. This role offers flexibility, with the option to work from home.
