Home Caring is a reputable and fast-growing provider of NDIS and Aged Care services, nationwide. We are seeking a experienced and skilled Case Manager to join our Home Caring Wyong team. This pivotal role is responsible in ensuring all services are delivered to meet the values, quality and service standard that our clients deserve.
The Key Responsibilities:
* Review and provide specialist input on client care plans, ensuring alignment with individual needs, goals, and evidence-based best practice standards.
* Advise care teams on complex care situations, including changes in client physical, cognitive, or mental health status.
* Identify and advise on solutions to complex scheduling challenges, including conflicts, gaps in coverage, or high-risk client scenarios.
* Coordinate and manage the scheduling of client visits and support worker assignments, taking into account factors such as availability, skills, geographic location, and client preferences.
* Develop, allocate and manage financial resources and budget utilisation and monitor surplus/deficit strategies.
* Conduct regular audits of Care Plans, budgets to ensure quality and compliance.
* Manage complaint escalations to a favourable conclusion or escalate where appropriate.
* Analyse service delivery data to identify trends, gaps, and opportunities for improvement, and develop actionable solutions that enhance performance and client outcomes.
What We’re Looking For:
* Demonstrated experience in Case Management (essential)
* Experience working within NDIS and/or Support at Home (Aged Care) (highly regarded)
* Ability to manage a mixed caseload across central cost participants
* Strong capability to work autonomously with minimal supervision
* Proven problem-solving skills and ability to navigate complex situations
* Excellent rapport-building and relationship management skills with participants, families, and stakeholders
* Confident working in a work-from-home (WFH) environment, with strong time management and self-organisation
* Drivers Licence, own vehicle and willingness and ability to travel the coast as required for client visits.
* Strong networking and stakeholder engagement skills, including building referral pathways
* Comfortable in fast-paced, regulated, or changing environments
* Proactive and committed to continuous improvement
