About the Role

The Fortify Health Care Services team is supportive, approachable and client-centred and also provides excellent staff training and development opportunities through our training academy.
We provide care and support to participants with complex health needs, including mental health, challenging behaviours and high-intensity support needs.
The Disability Support Worker/Enrolled Nurse is responsible for the delivery of safe and effective nursing care, under the direction of the Registered Nurse (RN).
Responsibilities
Adhere to positive behaviour support plans and implement documented strategies to support positive outcomes for participants.
Assist with high intensity support needs including diabetes management and subcutaneous injections.
Deescalate and provide mental health support; manage and provide a safe & calm environment.
Attend to personal cares, catheter flushes, medication administration and other high-intensity support requirements.
Support participants with daily supports including light domestic tasks, transport, meal preparation and companionship.
Use encouragement and motivation to support participants toward independence and personal goals.
Work closely with fellow team members to create a smooth, stable, and positive service environment; lend a hand to colleagues when needed.
Complete required documentation, including shift notes, incident reports and data collections via in-house support charts, and contribute to development, modification & evaluation of Participant Care Plans.
Build strong rapport with participants and stakeholders.
Attend to medical emergencies and provide first aid as required.

To be Successful in This Role
Current AHPRA registration as an Enrolled Nurse with no restrictions.
Relevant nursing qualifications for EN roles.
Cert III in Individualised support or significant experience in supporting individuals with disabilities.
Work a rotating roster with willingness to work night duty (if required) including inactive and active sleepovers.
Lived or worked experience supporting participants with complex health, mental health and positive behaviour support plan implementation.
Excellent technological, communication and time management skills.
First Aid and CPR certificate.
Relevant compliance checks, including NDIS Worker Screening Check & Blue Card.
Driver's licence and own vehicle.
