Our network is growing across Caboolture, Morayfield, Strathpine, Deception Bay, and Margate – now there's an opportunity for you to be part of a great team making a real difference in people's lives.**About the role**Working within a multidisciplinary team to support clients with moderate to severe mental health needs that are at risk of entering the acute care system if their symptoms are unmanaged, or who are stepping down from an inpatient episode of care to ensure they are well supported in the community through holistic and wrap around primary health and mental health care. You will provide a key leadership role within the team, supporting high quality clinical practice that supports the Staying Deadly Ways of Working, culturally appropriate comprehensive assessments, treatment, and intensive care coordination along with assertive outreach to meet clients emerging mental health needs. Your role will support a direct caseload as well as overseeing, support, and developing high quality, safe care across the team.Your day to day duties may include:* Support the Hub manager with the establishment, provision and review of effective, quality service provision in line with community expectations and the Staying Deadly Ways of working.* Manage an appropriate complex caseload using effective time management and prioritisation skills, including clinical and non-clinical responsibilities and the maintenance of accurate client records and documentation.* Lead professional and multidisciplinary team meetings including clinical case conference.* Maintain and develop appropriate referral pathways and stakeholder relationships including with community and acute mental health services.* Support staff through client issues and incidents, liaising with clients directly where issues are escalated.
