This is a uniquely rewarding opportunity to deliver holistic, wellness-focused assessments for older Australian, supporting independence and dignity both at home and in residential aged care settings, without shift work or weekend rosters.

What You'll Be Doing
* Conduct person centred, strengths-based assessments focused on wellness and independence.
* Work collaboratively with older people to develop personalised goals and support plans.
* Recommend supports such as assistive technology, community services, and social programs.
* Use the My Aged Care Integrated Assessment Tool.
* Work independently while remaining closely connected to a supportive and knowledgeable team.
About You
You're a confident, values-driven clinician who enjoys working autonomously while making a real difference in people's lives.
What you'll bring
* Experience in aged care and/or community health.
* Previous assessment or My Aged Care experience highly regarded, but not essential - training and support will be provided.
* A warm, confident and compassionate communicator who builds rapport with clients, families, and stakeholders.
* Confidence managing your schedule and working to KPIs.
* Knowledge of the Hume region, or a strong willingness to learn and connect with local communities.
* Well-developed digital skills and comfort working across multiple systems.
Qualifications & Checks
* Tertiary qualification as a Registered Nurse, Occupational Therapist or Physiotherapist.
* Significant experience working with older people (ideally 5+ years in the aged care or related setting).
* Willing to undergo a Nationally Coordinated Criminal History Check.
* Valid Australian Working Rights.
