About The Role
The Sustained Nurse Home Visiting (SNF) program provides coordinated, nurse-led home visits aimed at improving outcomes for vulnerable families and children. In this role, you will work in partnership with families to achieve goals and desired outcomes, while keeping the baby's wellbeing front of mind.
Your typical week involves structured home visits, active follow-up, and collaboration with families. You will participate in weekly case conferences and care discussions with the multidisciplinary team to ensure continuity and quality of care. Orientation to the SNF program will be provided, and ongoing support includes supervision from the Manager for Child and Family Services, NUM/Coordinator for SNF, CNC and CNE for Child and Family Health Services, Allied Health team members, and nursing colleagues. All SNF nurses are required to complete and maintain mandatory education requirements and undertake the Advanced Nurse Practice (ANP) Course, supported by an ANP Course Coordinator.

About You
You are passionate about making a positive impact and bring:
* Current Authority to Practice as a Registered Nurse and/or Registered Midwife with the Australian Health Practitioner Regulation Agency (AHPRA).
* Current qualifications in Child and Family Health Nursing and willingness to complete Advanced Nurse Practice course within 12 months of employment.
* Eligibility to drive in NSW and willingness and ability to travel for work purposes
* Enthusiasm and dedication to working with vulnerable families and children
* Ability to form therapeutic, respectful relationships while maintaining professional boundaries
* Strengths as a reflective practitioner, using a partnership and strengths-based approach
* Commitment to professional development, including completing the Advanced Nurse Practice (ANP) Course
* Collaborative approach and willingness to work within a multidisciplinary team
