Job Overview:
The role of Clinical Change and Transition Manager is a key position within our organization. The successful candidate will be responsible for coordinating and facilitating the transition program, ensuring seamless integration with existing systems.
This involves working closely with stakeholders to develop and implement strategies for change management. Additionally, you will work in partnership with other organizations to coordinate planning, development, and implementation of transition programs.
The ideal candidate will have substantial previous experience in clinical operations, change management, operational management, and/or project management roles within Health. They must demonstrate high-level skills and experience in change management and/or planning and development, advanced-level analytical and problem-solving skills, and the ability to provide high-level influencing, interpersonal, communication, and negotiation skills with managers and staff regarding Service Planning matters.
