Position Overview
We are seeking an experienced and dedicated Support Coordinator to assist NDIS participants in implementing their plans, connecting with services, and coordinating supports to build capacity and independence. This role requires strong communication, problem-solving, and case management skills.
Key Responsibilities
* Connect participants with relevant services and supports.
* Help participants negotiate your service agreements.
* Set up appointment bookings.
* Help participants to understand and make the most of their NDIS plan.
* Manage challenges in your support environment such as access to education, health or justice services.
* Reduce barriers that are preventing participants from implementing or using your NDIS plan as intended.
* Help build capacity.
* Provide support to strengthen a participant's ability to design and build their support, emphasizing linking a broader support system across the delivery of a complex service environment.
* Focus on supporting participants to direct their lives, not just their services, e.g. crisis and resilience development.
* Apply a time-limited outcomes-focused manner with stakeholders.
* Assist participants in connecting with NDIS supports, negotiating solutions with multiple stakeholders, and building their capacity and resilience.
* Assist participants in developing knowledge, experience and connections with their community and broader systems of support.
* Work with the participant to assist them in understanding plan funding and its purpose.
* Identify and understand participant confidence and skills assist participants in deciding what they want from services.
* Develop and design support solutions to meet participant outcomes.
* Assist the participant to identify and consider support options and linking the participant to the broader systems of supports.
* Where practical, create a support and action plan to facilitate participant plan implementation.
* Assist in resolving points of crisis and developing capacity and resilience within the participant's network.
* Coach the participant through challenges.
* Assist participants in preparing for the review and reporting of their achieved outcomes.
* Liaising with other health institutions and health personnel to improve and implement care to the clients.
* Manage rosters, administration and intake process.
* Organization and provision of education and community access plan and needs of people living with intellectual, physical, social, and emotional disabilities.
* Provide supervision and coaching to the Senior Support Workers and Support Workers in the team to enhance their practice and the outcomes for clients.
* Maintaining accurate record and progress of clients.
* Willing to travel across Australia as per the requirement of the company.
Skills & Qualifications
Essential:
* Experience in Support Coordination, Case Management, and experience in the field.
* Diploma of Community Services, Social Work, Psychology, Disability, mental health or similar.
* Strong knowledge of the NDIS, disability sector, community services, or mental health
* Excellent communication, advocacy, and organisational skills
* Ability to work autonomously and manage a complex caseload
* Strong documentation and report writing skills
* Understanding of trauma-informed, person-centred and strengths-based practice
* Current NDIS Worker Screening / Working With Children Check or willingness to obtain
* Valid driver's licence and reliable vehicle
Desirable:
* Experience working with psychosocial disability or complex needs
* Experience using CRM or case management systems
* Knowledge of local services across NT
