Are you an experienced clinician passionate about supporting older Australians to live with dignity, independence and choice?

We are seeking compassionate and skilled Triage and Clinical Assessors to join our Aged Care Assessment Service (ACAS) team. In this rewarding role, you will use your clinical knowledge and judgement to assess client needs, determine eligibility for aged care supports and help connect people with the right services at the right time.

About the Role

As a Triage and Clinical Assessor, you will:

* Conduct phone-based triage and clinical assessments through the My Aged Care portal
* Assess clients’ medical, cognitive, mobility and psychosocial needs
* Determine appropriate assessment pathways and support options
* Complete Comprehensive and Home Support Assessments using the Integrated Assessment Tool (IAT)
* Develop support plans and make referrals aligned to client goals
* Collaborate with multidisciplinary teams and stakeholders across the aged care sector
* Maintain accurate client records and ensure compliance with legislative requirements
* Support a positive ageing approach focused on wellness, independence and choice

About You

To be successful in this role, you will have:

* Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist or Social Worker
* Current AHPRA registration or eligibility for AASW membership
* Knowledge of the aged care and community care sector
* Strong clinical assessment and decision-making skills
* Excellent communication and stakeholder engagement abilities
* Strong administrative and organisational skills
* Experience using digital assessment tools and client management systems
* A genuine commitment to person-centred care and service excellence
