About the role

Full‐time contract opportunity with the Southern Adelaide Local Health Network at the Repat Health Precinct. Salary ranges are $130,190 – $136,010 p.a. (RN3) and $112,139 – $120,284 p.a. (AHP3) pro‐rata. The position is based in the community geriatric care setting and focuses on supporting older adults to remain independent, safe and connected.

Key responsibilities

  • Provide individual case management, care planning and coordination for older people living in the community.
  • Use independent clinical decision‐making to deliver expert, tailored care and implement therapeutic interventions.
  • Lead complex discharge planning and hospital avoidance strategies, including education, equipment provision, referrals and evaluation.
  • Support clients and carers by connecting them with appropriate community services and resources.
  • Build strong partnerships through effective communication with families, carers and other health providers.
  • Contribute to a highly responsive, integrated collaborative team culture focused on client‐centred care.

About you

RN3: Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and holds or is eligible to hold a current practising certificate, with at least 3 years post‐registration experience. Demonstrated competence in geriatrics and community nursing practice in accordance with relevant standards.

AHP3: Holds an appropriate degree‐level qualification and membership (or eligibility for membership) with the relevant professional association, and/or registration with the relevant board – Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Psychology, Social Work or Exercise Physiology. Demonstrated ability to assume responsibility for the provision of clinical allied health services, to assess and intervene with clients with a variety of disorders and disabilities, particularly those with cognitive impairment or dementia, and experience in clinical management and leadership within a discrete unit, including programme evaluation and service development.
