Part-Time Social Support Worker Opportunity

The role of the part-time Social Support Worker involves acting as a helper for dialysis patients and their families through three core functions.
1. Acting as a Social Support Worker for dialysis patients and their families.
2. Providing practical support for clients with complex needs as directed by the Social Support Coordinator.
3. Working collaboratively with the Social Support, Elder Care, and Clinical Services teams.
4. Assisting clients with:
o Personal shopping and banking.
o Attending appointments and meetings (medical, housing, Centrelink, legal).
o Visiting family members and participating in social and cultural activities.
o Preparing and cooking meals for social activities.
o Organising and participating in approved social activities.
o Facilitating personal grooming and care.
o Transporting clients to various activities or appointments.
o Supporting clients in advocating for their health, housing, financial, family, and cultural needs.
o Helping clients access and negotiate the service sector.
o Working alongside the Clinical Coordinator to support primary health care needs.
o Performing other relevant duties within the scope of training and capacity.
Key Qualifications:
- Skills and/or experience in Social Support Services.
- Open driver's licence and willingness to travel to remote communities.
- Current National Criminal History Check (within 6 months).
- Up-to-date COVID-19 vaccinations (2 doses plus booster).
- Willingness to obtain Food Safety Certificate, First Aid and CPR if not already held.
