Key Responsibilities
* Assessment and Case Management: Coordinate client care needs through comprehensive assessments and individualized case management plans.
* Support to Clinical Team: Collaborate with the clinical management team to develop and implement AN-ACC documentation and comprehensive assessments.
* Care Assistant Supervision: Provide guidance and support to Care Assistants to ensure high-quality patient care.
* Interdisciplinary Collaboration: Liaise with clients, their significant others, and multidisciplinary teams to achieve positive client outcomes.
