Position Overview: We are seeking a dedicated, compassionate, and open-minded Support Coordinator to join our team in the Barwon region. In this role, you will play a pivotal part in empowering individuals with disabilities by providing personalised, high-quality support throughout their NDIS journey. The ideal candidate will be highly organised, reliable, and genuinely passionate about working alongside people with disabilities. You will be responsible for ensuring that clients receive the right supports and services to help them achieve their goals, while fostering independence and promoting a sense of autonomy. Role Details: Type: Casual with the potential to become Full-Time Location: Work from home, with community visits. Fortnightly office days, and monthly team meetings in Melbourne and Geelong regions. Job Role Includes (but is not limited to): Supporting participants’ engagement with the NDIS Coordinating formal and informal supports Understanding mental health and navigating the mental health system Crisis management and coordinating with service providers Assisting with government agencies, medical, and disability-related appointments Report writing Networking and communicating with key stakeholders Working autonomously from home (this is a remote role) Traveling within your allocated caseload area, as well as monthly to Geelong Head Office or team meeting locations across Melbourne. Suburbs include, but are not limited to: Lara, Corio, Highton, Waurn Ponds, St Leonards, Bannockburn, South Geelong, Grovedale Applicant Criteria: MUST have experience or sound knowledge of NDIS Support Coordination or Outreach Support experience preferred SDA and SIL knowledge preferred Qualification in disability or community sector (e.g., Cert IV in Disability, Welfare, Mental Health) preferred Experience working with people with disabilities and psychosocial disabilities Ability to travel, including having your own car Working With Children Check (WWCC) High level of computer skills Outstanding communication abilities Evidence of NDIS worker screener clearance or in the process of obtaining clearance (includes a police check) Ability to work autonomously from home Registered NDIS service provider www.libertydisabilityservices.com.au
