* Provide virtual supports and short-term psychological interventions, connecting participant to longer-term services and supports
* Expand your complex case management and care coordination skills in the virtual environment

* Be a part of a multidisciplinary team in providing in evidence-based best practice and high quality services

Joining us, you'll work alongside a team of dedicated individuals who are focused on supporting people experiencing a suicidal crisis within the community, directly impacting individuals in an impactful and meaningful way.

Job Description

We have an exciting opportunity for an experienced and passionate Senior Support Coordinator to join a team in our Care Connect program, across the Central Coast and Hunter Valley.

* Full-time (Mon-Fri) from 11 am to 7 pm

* Work from home location, with ability to attend our offices in Central Coast and/or Hunter Valley on occasion

* Initial contract to 31 December 2026, with opportunities to extend

Care Connect is an aftercare service supporting people experiencing a suicidal crisis or who have attempted suicide. The service focuses on supporting people and their care givers to connect with formal and practical support to assist with recovery.

We are looking for a passionate individual with a desire to make a difference. The successful candidate will have:

* Tertiary qualifications or equivalent experience in mental health, psychology, social work, social science or social welfare, health promotion or similar
* 3-5 years' experience in a suicide prevention / aftercare or mental health setting
* Expertise in managing complex and higher risk people and delivering brief psychosocial interventions using evidence-based approaches.
* Demonstrated experience in providing virtual care and demonstrated proficiency in digital literacy, cybersecurity and navigating virtual platforms.
* Ability to work from home, in a private confidential space, most days of the work week.
* Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people to ensure effective relationship
* Knowledge of the community services sector in the Hunter Valley and Central Coast is desirable.
