Base pay range
$42.16/hr - $45.36/hr
Location: South West Sydney region,M5 Corridor ( Liverpool / Fairfield / Bankstown)
Permanent: Full -Time (Max-Term until December 2025) | 1 WFH day/week
Program: C2BME
HPA Level 2 + Salary Packaging (reduce your taxable income)
About the program :
The "Continuing to be Me" (C2bMe) program of Uniting is a comprehensive in reach person centred model of care designed to enable residents living within aged care facilities with mental concerns to access mental health services like those available in the community.
C2bMe offers low and medium intensity interventions through therapeutic groups & individual clinical care led by skilled allied health professionals and supported by RACHs staff.
About the Role
As a Clinician, you will independently deliver comprehensive, evidence-based care, including clinical coordination and both psychological and psychosocial interventions. You'll provide individual and group therapy to support the mental health and wellbeing of older adults. In addition, you will contribute to capacity-building by delivering mental health training to care staff.
About you in the role
* Provide therapeutic intervention to older individuals experiencing mild to moderate mental health issues.
* Conduct assessments, including bio-psychosocial and risk assessments, and track outcomes.
* Facilitate group therapy, such as art and reminiscing therapy.
* Attend regular clinical review meetings, group supervision (monthly), and operational supervision (every 6 weeks).
* Work autonomously, managing your own schedule and caseload.
What you need to succeed
* Qualifications in Social Work, Psychology, Nursing (RN), Occupational Therapy, or Counselling.
* Registration with relevant professional bodies (APA, PACFA, or AASW).
* 1–2 years' experience in mental health intervention.
* A valid driver's license and own vehicle.
* Familiarity with electronic medical records.
* Ability to work autonomously and stay organised
