Through their specialised care and heritage programs, our client ensures every generation feels supported, valued, and proud of their identity ABOUT THE ROLE The Care Partner works in partnership with participants and the Clinical Home Care Team Leader to co-design a plan of care, based on the participant's goals, preferences and assessed needs, whilst promoting functional independence, wellness and lifestyles choices. The Care Partner will liaise with internal and external stakeholders to ensure positive participant outcomes within participants allocated Support Plan budget. The Care Partner is primarily responsible for the ongoing coordination and management of a comprehensive range of services and supports provided to an allocated group of Support at Home participants. This will be achieved through building partnerships with participants and their families, and the Home Care team. KEY POSITION RESPONSIBILITIES Service Delivery and Participant Care Undertakes initial assessment and develop personalised Care Plans to support participants in achieving their goals and living independently in their own homes and community Develop, implement and monitor participant directed care plans in collaboration with participants Maintains professional relationships with participants, family representatives to facilitate regular reviews and adjustments to Ca-re Plans as participants needs change. Provide participant directed case management services to participants acting as the main point of contact between participants, their informal carers, Home Care Workers and external service providers Establishes and maintains positive working relationships with all members of the Clinical Care team and with the broader Aged Care Service Provision and Rostering team. Setting up and maintaining participants Support at Home Maintain a caseload. Support at Home participants. Reporting, System and Analytics Undertake six monthly reviews with participants and update relevant Ensure details on the My Aged Care portal are up to date Monitor participant's budget/sand take early corrective action to prevent a budget Ensuring timely documentation of participant information in accordance with relevant legislation and funding requirements on CRM Lookout. Ensure details on the My Aged Care portal are up to date Incident and hazard reporting in accordance with organisational procedures and reporting up to Quality, Risk and Compliance Team Regularly monitor and review participant progress, adjusting care plans as needed to optimize Ensuring required documentation is appropriately maintained to accurately meet statutory and organisational requirements Report Complaints and Compliments in line with organisational policies and procedures Risk Management and Compliance Ensure compliance with all relevant legislation, standards, and funding guidelines, including Aged Care Quality Standards.
