THE ROLE Targetted Care Packages Planner The Targeted Care Packages Planner plays a key role within the North Division Out-of-Home Care Targeted Care Packages Team by supporting the implementation, coordination and monitoring of activities associated with individualised and targeted packages to improve outcomes for children in out-of-home care and prevent children entering care. You will use your clear understanding of the placement requirements of children and young people, negotiation skills and ability to work with key internal and external stakeholders is to implement tailored supports to meet the needs of individual children, their families, and carers, providing flexible and innovative placement prevention and out-of-home care solutions for children. The Targeted Care Packages Planner will be required to contribute to the implementation, coordination and monitoring of activities associated with individualised and targeted support packages. The position reports to the Senior Targeted Care Packages Planner, Flexible Packaging, Care Services Operations Support, Division-wide Service Operations, North Division. ACCOUNTABILITIES INCLUDE * Undertake and manage active Targeted Care Package (TCP) processes for children and young people, to support their transition from residential care or prevent their entry. * Support to maintain client and financial records (including IFAMS) regarding status and movement of individual young people in the program and utilise data to generate client level funding commitments, reports and annual acquittal reconciliations. * Prepare, present or contribute to comprehensive reports, briefs, presentations and correspondence on a range of complex matters, related to the TCP initiative.
