Job Opportunity in Aged Care
We are seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for delivering person-centred case management to our clients.
About the Role:
The successful candidate will have experience in delivering quality, client-focused case management and a thorough understanding of the various funding sources available in the Aged Care sector. They will also possess tertiary qualifications in Community Services, Case Management or related disciplines.
About You:
* Excellent communication skills and ability to work effectively with clients and stakeholders
* Strong analytical and problem-solving skills
* High level of computer systems proficiency, including data entry
* Ability to develop care plans for complex clients
* A current and valid Australian driver's license
* A reliable, comprehensively insured vehicle
* Current First Aid qualifications (or willingness to obtain before commencement)
