What you'll be doing

The primary purpose of this position is to lead the SCHN palliative care pharmacy hospital-based service at CHW and SCH.

The SCHN Lead Pharmacist – Palliative Care provides comprehensive medication management, clinical care and care coordination to children and young people across SCHN/NSW who have end of life, symptom control and palliative care needs.

The SCHN palliative care pharmacy service provides high level, evidence based, tertiary clinical pharmacy services to patients and families across the continuum of care within the paediatric palliative care service.

The role will be supported by the Network Director of Pharmacy, SCH and CHW Directors of Pharmacy and the SCHN Palliative Care Head of Department and Program Manager. This position is a network position and reports to the Network Director of Pharmacy, with line reporting based on selection of a site preference, SCH or CHW.

Supervise and lead education and research activities in paediatric pharmacy training programs to improve pharmacy services capability in palliative care.

* Answers to the selection criteria in the Position Description:

* Current registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA) with no restrictions on practice.

* Recent, extensive and varied clinical experience as a hospital pharmacist with a strong preference for paediatrics' with Palliative Care experience and post graduate qualifications.
* Demonstrated high level specialised knowledge and skills of drug use in paediatric patients and management of children with complex, chronic care and palliative conditions.
* Personal ability to withstand the challenging care environment of paediatric palliative care medicine.
* Demonstrated evidence of being professionally active indicating self-motivation along with competence in managing competing priorities to deliver quality outcomes.
* Participate in weekends, on-call and departmental rostering requirements of the SCH or CHW Pharmacy Department and Network pharmacy service if required.
* Consistently demonstrates behaviours that reinforce CORE values of our organisation; Collaboration, Openness, Respect and Empowerment.
