The Palliative Care Nurse Practitioner will be a valued member of the Wodonga Community Palliative Care service to provide clinical leadership, mentoring, and undertake specialist palliative care clinical consultation to enhance the quality of life for a person with a life limiting illness. As a permanent part‐time role, the practitioner will work as part of a multidisciplinary team including Clinical Nurse Specialists, Occupational Therapist, Social Worker, Grief and Bereavement worker, and Volunteer Coordinator, providing holistic care to patients and their families in the community setting.
Responsibilities
• Provide direct and indirect specialist palliative care for patients and families in the community setting.
• Mentor and support team members and contribute to the delivery of palliative care education and training opportunities.
• Identify and participate in quality improvement activities and research opportunities.
• Engage with and promote reflective practice across the nursing workforce and provide clinical supervision.
Qualifications
* Current registration & endorsement with AHPRA as a Nurse Practitioner of Palliative Care
* Demonstrated ability to work collaboratively and provide flexibility within the multidisciplinary community palliative care team, as well with local palliative care consultants and specialists, general practitioners and other health care providers
* Current driver's licence
