Care Designer
We are seeking an experienced Care Designer to join our team. As a key member of our staff, you will play a vital role in ensuring the well-being and happiness of our clients.
In this full-time position, you will work in a home care environment, providing some flexibility and availability to work occasional evenings, weekends, and on-call as required.
Responsibilities:
* Design client-centered care plans that meet their unique needs and preferences.
* Meet with potential clients to discuss their options for premium home care services.
* Oversee client clinical care and maintain accurate documentation records.
* Collaborate with caregivers and nurses to ensure high-quality care delivery.
* Contribute to the development and management of our growing team of caregivers and nurses.
Requirements:
* AHPRA Registration
* Registered Nurse
* CPR & First Aid Certification
* Current Victorian Drivers License
* National Police Check (Aged Care/Vulnerable Persons Sector)
* Working With Children Check
* Disability Workers Exclusion Scheme Approval
* NDIS Worker Orientation Module Completion
