Role Overview
This role is centered around delivering high-quality care management activities that enable older individuals to live safely and independently in their homes.
The Care Partner will work collaboratively with participants, supporters, and interdisciplinary teams to coordinate personalized support.
The focus will be on relationship-based care management, planning, monitoring, service coordination, and education, all aimed at promoting wellness and culturally safe care.
Key Responsibilities

Required Skills and Qualifications
* Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience.
* Experience in care coordination, case management, or similar roles in aged care, health, or disability services.
* Solid understanding of the Support at Home Program, Strengthened Aged Care Quality Standards, and aged care rights.
* Competence in interpreting clinical guidance and escalating appropriately.
* Strong communication, empathy, and cultural competence.
