The Transitional Rehabilitation Program (TRP) is a community-based rehabilitation team that provides specialist rehabilitation and transition support to people with new spinal cord injuries (SCI) immediately after discharge from hospital. While most TRP patients are referred directly from the Spinal Injuries Unit, Princess Alexandra Hospital, patients may also be referred from non-specialist rehabilitation services and acute hospital wards across Queensland.We are Metro South HealthWe are the major public healthcare provider for Brisbane's south side, Logan, Redlands and the Scenic Rim. We are better together .Join us at Metro South Health and be part of a world-class, dynamic and growing health service that embraces technology, excellence in health care, teaching, research and empowering our people to be the best in serving our community.About The RolePrepare and /or implement individual or group treatment programs for patients under the supervision and direction of the therapists and report on patient performance as required.Support person centred care approaches inclusive of patients' psychological, social, emotional, spiritual and physical care needs.Report any adverse circumstances with respect to facilities, equipment and people safety and implement remedial action as directed.Maintain responsibility for personal and professional development by participating in supervision, training and development activities, supervision and performance reviews and appraisals.Provide a range of individual patient interventions as delegated by the AHP and work under the direction of AHPs.
