Senior Social Worker - Assertive Community Management
We are seeking a skilled Senior Social Worker to join our team in an Assertive Community Management role. In this position, you will lead comprehensive psychosocial assessments and develop personalized treatment plans addressing complex alcohol and drug challenges.
* This is a fixed-term, full-time position playing a pivotal role within our service, delivering expert social work support to patients, families, and carers throughout their care journey.
* You will coordinate safe, effective discharge planning, ensuring seamless referrals to community supports and facilitating access to appropriate short- and long-term accommodation or placement options that best support patient recovery.
* As a strong advocate, you will manage complex applications and referrals to key agencies such as NCAT Guardianship Division, NDIS, Housing NSW, Department of Home Affairs, My Aged Care, LTCS, and CTP claims, ensuring patients and families receive holistic care and vital resources.
About You
To be successful in this role, you will bring a Bachelor of Social Work and eligibility for AASW membership, backed by solid clinical experience in tertiary health settings. You should be confident navigating challenging areas like alcohol and drug interventions, harm minimisation, and co-morbid health issues.
