Job Title: Community Health Professional
We are seeking a skilled and dedicated individual to join our team as a Respiratory Liaison in Community Health at the Coffs Harbour Health Campus.
Key Responsibilities:
* Provide safe, efficient, and effective nursing care that reflects current standards of nursing practices and policies.
* Work collaboratively with multidisciplinary teams to coordinate patient care and achieve desired outcomes.
* Develop and implement comprehensive care plans that address the unique needs of patients.
* Conduct thorough assessments and provide appropriate interventions to ensure optimal patient outcomes.
* Communicate effectively with patients, families, and healthcare professionals through verbal and written communication.
* Participate in quality improvement initiatives to enhance patient care and services.
Requirements:
* Current registration with the Nursing and Midwifery Board of Australia.
* Proven nursing skills, knowledge, and ability relevant to community settings.
* Demonstrated clinical problem-solving skills to maximize patient care.
* Effective clinical communication skills through the use of information technology and interpersonal skills.
* Ability to plan and coordinate patient care in a multidisciplinary team environment.
* Understanding of management of clients with complex needs in community settings.
* Valid NSW driver's license.
