As the Practice Nurse, you will be responsible for providing high level care to patients in a primary health care setting. Your role will be required to work collaboratively with other clinicians and liaise with onsite General Practitioners and external providers to achieve the best outcome for patients. You will provide medical care to patients to address health concerns, promote holistic wellbeing, engage in health education and build collaborative relationships to support the community's overall health and wellness.
This incredible Clinic is an Aboriginal Community Controlled Health Organisation based in Forbes, NSW. It provides holistic, culturally appropriate, comprehensive primary health care services, mental health support, social support services and allied health to Aboriginal and non-Indigenous people. They provide a range of programs to enhance Aboriginal and Torres Strait Islander people's overall wellbeing including health related programs, programs to strengthen community connection, mental health awareness and more. The clinic is supported by General Practitioners, Nurses, Aboriginal Health Practitioners, Community Support Team including Social and Emotional Wellbeing, Family Violence, Integrated Team Care and various visiting allied health services including Optometrist, Audiologist, Cardiologist, Podiatrist, Dietitian, Gastroenterologist. YG has visiting Counsellor, Mental Health Nurse, Psychologist and refers to Psychiatry.
Your Key Accountabilities
* Ensuring the delivery of culturally appropriate primary health care to the Aboriginal and Torres Strait Islander community.
* Liaising and work collaboratively regularly with GP's for patient health care, completing appropriate referrals for patients to appropriate allied health, support services and mental health support.
* Provide patient care including health checks, assistances with the management of Chronic Disease Care, clinical observations, triaging patients, patient immunisations, dressings, spirometry and more.
* Managing patients recalls, reminders and follow up for patient health outcomes.
* Planning and implementing health care initiatives including providing education to patients and families regarding good health, illness prevention, early intervention.
* Completing regular health checks for patients with a priority being Aboriginal Health Checks (715's).
* Coordination of infection control processes and other relevant policies and procedures relevant to a health service.
* Ensuring compliance with accreditation standards and contribute to clinical initiatives.
* Assessing and improving the overall health care of patients within the community with a priority being Indigenous patients and outcomes.
Qualifications & Experience Required
* Bachelor of Nursing
* Minimum 5 years' experience as a Registered Nurse
* Current AHPRA registration with no restrictions
* Immunisation Certificate
* Primary healthcare/general practice experience preferred but not essential.
* Preferred understanding of issues that relate to the wellbeing of Aboriginal and Torres Strait Islander families.
