We are seeking an experienced Community Nursing Lead to establish and lead a new community nursing service, delivering high-quality care to clients in their homes in Melbourne. This is a unique opportunity to build a service from the ground up, shaping clinical frameworks, governance, and team culture while also remaining actively involved in frontline nursing care. You will combine hands-on community nursing with clinical leadership and service development, working closely with senior stakeholders to design and scale a safe, evidence-based, and client-centred service. Key Responsibilities Deliver high-quality community nursing care including assessments, care planning, and clinical interventions Lead the design and implementation of a new community nursing service model Develop clinical policies, procedures, care pathways, and governance frameworks Ensure compliance with AHPRA standards, legislation, and best-practice guidelines Provide clinical leadership, supervision, and mentoring to nursing staff Drive quality improvement, audits, incident reviews, and outcome monitoring Support recruitment, onboarding, and development of a growing nursing team Collaborate with leadership, care coordination, and technology teams to integrate service delivery Contribute to service growth, innovation, and continuous improvement About You Registered Nurse with current AHPRA registration (essential) Minimum 5 years’ experience in community, primary, or home-based nursing Strong clinical leadership and team supervision experience Proven ability in clinical governance, policy development, and quality improvement Comfortable balancing frontline clinical work with leadership responsibilities Strong communicator with the ability to engage clients, families, and stakeholders Organised, adaptable, and comfortable working in a fast-paced, growing service environment This is a rare opportunity to shape a new community nursing service from inception, influence clinical standards, and build a high-performing team while maintaining meaningful hands-on patient care.
