The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics, Primary Health Care, Counselling, NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.

SWAMS is dedicated to enhancing employment opportunities and outcomes for Aboriginal and Torres Strait Islander people. This position is designated under Section 50 of the Equal Opportunity Act 1984 (WA), and as such, only Aboriginal and Torres Strait Islander people are eligible to apply.

We're looking for a full‐time, enthusiastic, and dedicated person who identifies as Aboriginal or Torres Strait Islander to join our team as a Case Worker (50d position), Out of Home Care. In this rewarding role, you'll work closely with carers, families, children and young people who are entering or currently in out‐of‐home care arrangements.

About the role

The Caseworker will be providing and ensuring, as part of a holistic team, comprehensive case management support to families, carers and children entering or in foster care placements in line with SWAMS policies and procedures, Department for Communities expectations and statutory and legislative requirements. You will supervise, assess and support carers to ensure placements are safe, nurturing and child focused and facilitate keeping children connected to culture and maintaining a relationship with birth families. You will have mentor support from the Senior Case Worker in undertaking this role. You will also be working with Transition Coaches to support the smooth transition of young people in care to their independence.

* Knowledge of the Children and Community Services Act 2004.
* Basic knowledge of developmental needs of children and young people.
* Demonstrated knowledge and understanding of Aboriginal culture and contemporary issues affecting Aboriginal People, and the ability to communicate sensitively and effectively with communities, families, children and young people.
* Strong written skills, including report writing.
* Demonstrated ability to manage own time and work resources.
* Ability to work effectively as a team member.
* Ability to liaise with relevant government and non-government professionals, community groups, services, and agencies and present and report information to internal and external stakeholders.
* Demonstrated research, analytical and problem-solving skills, in relation to case management.
* Possession of or substantial progress toward a qualification in Social Work, Youth Work or other relevant qualification or substantial lived experience relevant to the position and willingness to complete further education.
