Job Description:
The Department of Health is seeking experienced and dedicated Clinical Nurse Consultants to join our team as Residential Aged Care Home (RACH) Liaison Nurses. These roles are pivotal in ensuring seamless transitions for patients moving between hospital and residential aged care homes, providing expert clinical advice, and fostering strong partnerships with aged care providers.
This role provides an exciting opportunity for RACH Liaison Nurses in Tasmania's hospitals, working with Hospitals South, North or North West Integrated Operations Centres (IOC) and Hospital Aged Care Liaison Teams (HALT). The successful applicant will work in partnership with IOC or HALT Leadership Teams, clinical staff, and Private Providers to assist patients and their families navigate the pathway from hospital to a RACH.
Key Responsibilities:
* Provide expert clinical leadership and advice within an interdisciplinary team.
* Conduct comprehensive patient assessments and coordinate care plans for transfers to RACHs.
* Collaborate with RACH providers and General Practitioners to ensure continuity of care.
* Utilise and evaluate models of care and service delivery based on evidence-based practices.

Required Skills and Qualifications:
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
* Holds or is working towards a relevant postgraduate tertiary qualification and/or recent extensive nursing experience relevant to the role.
