The Role:
* Provide exceptional in-home care and community access to individuals requiring assistance with daily life tasks
* Liaise closely with multidisciplinary teams to develop tailored support plans that promote empowerment and personal growth
* Foster inclusive environments that respect diversity and individuality, promoting social participation and community engagement
Your Qualifications & Skills:
To be successful in this role, you will need:
- Minimum qualification in Certificate 3 or 4 in Mental Health or Disability; experience across diverse disabilities types desirable.
- Expertise required includes expertise working across hoisting/lifters, medications administration and supervision and guidance regarding Community Access,
