Job Description
This is a full-time contract position working 38 hours per week from Monday to Friday with flexibility to work from home twice a week.
Key Responsibilities:
* Explore mainstream, community, advocate and informal options for participants.
* Support participants in establishing their MyID account and navigating the NDIS Portal to negotiate services and support agreements.
* Negotiate quotable supports as decided by the participant and arrange assessments to determine funding requirements.
* Liaise with Plan Managers on funding, support types and payments etc.
* Link participants to mainstream and/or community services such as housing, education, transport and health as required.
* Sustain and enhance participants' capacity to self-direct and manage supports.
* Diploma or higher tertiary qualifications in disability/community services/social sciences/allied health.
* Knowledge of the NDIS Code of Conduct, Practice Standards, Act 2013 and rules.
* Understanding of the complex needs of people with disability and their families/nominees/authorized representatives.
* Experience with effective and contemporary disability support models and person-centred approaches.
* Proven ability to establish partnerships and achieve outcomes with communities and organisations.
* Proficiency with Microsoft Office and client database systems.
* Excellent report writing, time management and record-keeping skills.
