About the role
At The Salvation Army, we believe in a future where long-term homelessness is not inevitable. Through ourJourney to Social Inclusion (J2SI)program, we walk alongside those who have experienced chronic homelessness, supporting them to build lasting stability, dignity, and connection. J2SI is intensive, evidence-based, person-centred, and trauma-informed. Our team is empowered to make a deep impact—offering wraparound support over three yearswith low caseloads, meaningful relationships, and time to truly understand and empower each participant. Now, we're inviting a compassionate, resilient, and skilledCase Managerto join us. Reporting to the Journey to Social Inclusion (J2SI) Team Leader, this is apermanent full-timeposition, based in Geelong, VIC. Compensation is in accordance withSCHADS Award Level 5.
You will successfully
Work intensively with a small caseload of individuals experiencing chronic homelessness Deliver outreach-based, long-term case management and service coordination Build deep, trusting professional relationshipsthrough assertive and persistent engagement Empower clients to access housing, health care, employment, and social inclusion opportunities Partner with ACCOs and First Nations workers to provide culturally safe support Capture meaningful progress through robust outcomes measurement and trauma-informed frameworks Be a key contributor to sector innovation, service collaboration, and evidence-based practice
You will have
A relevant tertiary qualification in Social Work, Psychology, Community Services or related field Demonstrated experience working with people with complex needs—homelessness, mental health, substance use, family violence Experience inoutreach, trauma-informed care, case management, or advocacy A values-driven, culturally aware approach to supporting people from all backgrounds A belief in thepower of persistence, trust, and human connection A current Employee Working with Children's Check A current VIC Driver's Licence
