About the Role
The Support Space is seeking compassionate and experienced female Disability Support Workers to provide individualised support to people living with Motor Neuron Disease (MND) in the Collie area.
Key Responsibilities
* Provide 1:1 support to participants with MND in their homes and the community.
* Support daily living activities (ADLs) and routines.
* Maintain clear, accurate documentation and progress notes.
* Support participants in community engagement and meaningful recreational activities.
* Assist with domestic duties such as cleaning, meal preparation, and laundry.
* Deliver high-intensity care for participants with complex needs.
* Transport participants to appointments and social activities.
About You
You are patient, adaptable, and passionate Support Worker, who is experienced in providing personal care and comfortable supporting participants during end of life care, with sensitivity, dignity, and emotional resilience.
Essential Requirements
* Current WA driver's licence and reliable, insured vehicle.
* National Police Clearance (obtained within the last 6 months).
* NDIS Worker Screening Check.
* Working with Children Check.
* Eligibility to work in Australia.
* Available to work a range of shifts including short shifts.
* Strong communication and documentation skills.
* Reliable, flexible, and confident using mobile apps for communication and documentation.
