You will work within the **Community Continuing Care Division**, providing programs such as: * Aged Care Assessment * Community Allied Health and Carer Support * Chronic Disease Management and Diabetes Services * Outpatient Rehabilitation * Geriatric Evaluation Management (including home-based models) * Allied Health Services across 12 disciplines, with strong governance and clinical support * Eligibility for registration with AHPRA * **FRACP (Geriatric Medicine)** or overseas equivalent * IMGs must hold or be eligible for specialist recognition in Australia
