Lead Mental Health Practitioner
About the role:
We are seeking a skilled professional to deliver advanced clinical nursing care and leadership in our Wheatbelt service. The successful candidate will provide person-centred evidence-based care to individuals living with severe but stable mental health conditions.
Key Responsibilities:
* Deliver high-quality, recovery-focused case coordination that strengthens continuity of care across health and social support networks.
* Conduct comprehensive mental health assessments, triage, and complete risk assessments.
* Develop, implement, and review collaborative care and recovery plans with clients and carers.
* Provide clinical case management, crisis intervention, and therapeutic interventions as required.
* Offer clinical guidance, mentoring, and professional supervision to team members delivering coordinated care.
* Contribute to service development, health promotion, and continuous quality improvement initiatives.
* Ensure accurate and timely clinical documentation in line with legislation, standards, and policies.
Required Skills and Qualifications:
The ideal candidate will be an experienced Registered Nurse with a strong background in mental health and a passion for providing quality healthcare for regional clients. They will bring both clinical expertise and coordinated recovery-based care experience, as well as demonstrated leadership capability and current registration with the Nursing and Midwifery Board of Australia (AHPRA).

This is a part-time position located in Northam, WA. The successful candidate will work collaboratively with a multidisciplinary team to deliver high-quality care and services to clients living with severe but stable mental health conditions.
Essential Requirements:
* Current registration with AHPRA
* Tertiary qualifications in Nursing with post-graduate qualifications in Mental Health Nursing or related discipline
* Experience in mental health nursing, ideally with a focus on care coordination
* Skilled in assessment, care planning, case management, and crisis intervention
* Excellent communication, negotiation, and interpersonal skills
* Ability to work autonomously and collaboratively within a multidisciplinary environment
* Sound knowledge of recovery-oriented practice and relevant mental health legislation
* Current WA driver's licence and willingness to travel for outreach work
Desirable Qualifications:
Familiarity with local community health and social service networks; experience providing clinical supervision, mentoring, or capability building; and knowledge of relevant mental health legislation.
