In this newly created role of Clinical Care Partner, you will be providing relationship-centred clinical case management to a diverse client base, ensuring needs are met through comprehensive assessment, collaborative care planning, and support that promotes independence, choice, and quality of life.
This role requires travel around the Coffs Harbour area as well as an "in office" day each week.
Day to Day This Role Looks Like
Delivering relationship-centred care aligned with Catholic Healthcare's Inspirit model
Providing comprehensive support during client contact to resolve any additional outstanding matters, provide welfare checks, review of current services and monitoring for any change in circumstances
Triaging and coordinating the provision of care management and services relating to short term pathways such as end-of-life and restorative care
Using client-initiated contact points to review care plans collaboratively with clients and families to optimise quality of life
Monitoring clinical indicators in real time and identifying risks to clients' health, safety, wellbeing and managing risks to prevent deterioration
Collaborating with clinical and operational teams to plan and evaluate care, ensuring high satisfaction and proactive issue resolution
Undertaking clinical investigations to identify root causes and to determine the appropriate action when incidents arise, including communicating with key stakeholders and recommending improvements
Conducting comprehensive client clinical assessment and screening per organisational procedures at time of onboarding, annual review or as required
Ensuring full utilisation of client budget and fee for service to meet needs as appropriate
Supporting the review process with My Aged Care when client needs result in a greater level of funding required
Providing advice to Care Partners, Care Coordinators and other internal stakeholders to support sound clinical decisions and overall quality of care management
Providing supervision and clinical oversight to Enrolled Nurses and/or junior clinicians
Understanding and adhering to the Aged Care Standards in all interactions, practices and documentation
To Be Successful In This Role You'll Need
Completion of tertiary qualifications in nursing or similar (Bachelor of Nursing) with a minimum of 3 years nursing experience
Current Registered Nurse registration with AHPRA
Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards
Strong clinical knowledge and understanding of the ageing process, particularly age-related health issues
Care management experience relating to assessment, care planning, goal writing and reviewing
Strong coaching / training skills with the ability to positively influence other healthcare workers
Effective conflict resolution skills with a high emotional intelligence
Strong customer-centric approach to business
Ability to work independently and as part of a multi-disciplinary team
Excellent digital literacy, including Microsoft Outlook and Excel
Demonstrated knowledge and understanding of workplace health and safety
