The Federal Government appointed Country SA PHN to enhance the delivery of focused and appropriate health services to the rural communities of South Australia.

The goal of the organisation is to improve the efficiency and effectiveness of health services for people particularly those at risk of poor health outcomes, improve the coordination of health services, and increase access and quality support for people.


Job Description

The Role

The Aged Care Support Officer role contributes to the implementation, delivery, and evaluation of a range of projects and strategies in response to the Australian Government's response to the Royal Commission into Aged Care Quality and Safety measures in country SA.

Reporting to the Senior Manager Strategic Engagement, this role works collaboratively with relevant managers and teams to strengthen relationships and alignment across shared areas of work. This role will encompass administrative and digital requirements and support the Program Lead Aged Care Integration in fostering strong aged care stakeholder relationships. The role is collaborative and supports a coordinated approach to engaging stakeholders, enhancing service delivery, and improving outcomes for aged care target populations.

The Aged Care Support Officer will be responsible and accountable for delivering the following activities in alignment with CSAPHN's Strategic Priorities :


Priority 1: Commission effective and efficient services that deliver health equity

* Contribute to CSAPHN planning and commissioning by identifying community needs and informing the design, delivery, and evaluation of aged care programs.
* Support the delivery of commissioned education, training, and promotional initiatives across the aged care sector to improve health outcomes for older people.

*This is not a training provider role


Priority 2: Facilitate local capacity for the delivery of responsive, sustainable services

* Support and work collaboratively with the Program Lead Aged Care Integration.
* Support the planning, implementation, and optimisation of digital health solutions (e.g. My Health Record, telehealth platforms), providing advice and training to aged care professionals.
* Work collaboratively across teams/workgroups, particularly with the Primary Care and Digital Support workgroup, to align activities and share expertise.
* Build and strengthen relationships between primary care, aged care, hospital, and community providers to enhance coordination and service integration.
* Promote innovation, sustainability, and continuous improvement across aged care programs.


Priority 3: Improve people's experience of primary health care

Ensure programs enhance person-centred care, care coordination, and digital experiences for both consumers and providers.

* Support the engagement of stakeholders to identify local needs, barriers, and opportunities for improvement.
* Support evaluation and feedback processes to capture experiences and inform continuous improvement.
* Apply evidence-based community engagement practices and collaborate across CSAPHN portfolios to support consistent, high-quality primary care experiences.


Priority 4: Practice responsible and effective corporate and clinical governance

* Support monitoring, evaluation, and reporting of aged care activities to ensure accountability and compliance.
* Support the coordination, growth and delivery of the Residential Aged Care Network (RACN) meetings and related stakeholder engagement activities.
* Contribute to the preparation of reports and updates for internal and external stakeholders, including the Australian Government.
* Work flexibly across aged care projects to support seamless coordination and continuation of key functions.

Other duties:

* Attend and present at meetings, seminars, and forums when required.
* Collaborate with the Clinical Councils and Community Advisory Committees as required, providing input within the confines of this role.
* Support the organisation to embed reconciliation initiatives into business and our everyday work in line with our CSAPHN Reconciliation Action Plan.

Desired Skills and Experience

Qualifications:

* A relevant tertiary or vocational qualification (or equivalent experience) in project support, health promotion, public health, or a related field, with experience or familiarity in Aged Care or general practice desirable.

Experience / Knowledge/ Skills / Abilities:

* Demonstrated skills and experience in supporting project delivery and program outcomes.
* Demonstrated understanding of primary health care in country South Australia.
* Experience in a health-related role - this may include voluntary work.
* Demonstrated ability to build rapport and connections with a wide range of key internal and external stakeholders to ensure best collective outcomes and solutions.
* Project experience whilst working under pressure, to timelines, achieving and reporting against outcomes.
* Experience in developing, establishing and/or maintaining projects and ability to develop effective systems and task coordination.
* Excellent written and verbal communication with strong interpersonal skills and the ability to communicate effectively at all levels.
* Strong digital capability and understanding of digital health systems.
* Ability to work with minimal supervision, as part of a team and across business areas, functions, and organisations.
* Proactive, well organised, with excellent time management and problem-solving skills.
* Proficiency in word processing, spreadsheet, project, and presentation software.
* Ability to be adaptable and able to adjust to changing project circumstances.
* Possess strong analytical skills, be able to use data to monitor and evaluate project progress.

* Demonstrated knowledge or clinical experience of aged care, particularly with Residential Aged Care Homes or General Practice
* Sound health literacy with knowledge of primary health care principles and practice.
* An understanding of ethics and confidentiality issues particularly in relation to the health and medical professions.
* Demonstrated experience in living in and working with rural communities, with a good understanding of country South Australia.
